If you have scheduled your appointment with Thompson Oncology Group, welcome to our practice and thank you for trusting us to be part of your health care team. Sharing accurate information with your care providers is very important, but trying to complete forms and recall your health history and medications as you sit in the lobby before your appointment can be overwhelming.

Please take advantage of the opportunity to complete your paperwork in advance and bring it with you. This option allows you to take your time and make sure the information your provide is accurate and complete. When it is time for your appointment you can focus on your meeting with your physician and any questions you may want to ask.
